DJ Lupe Service agreement A $50.00 deposit is required for all bookings. Lupe, do hereby personally guarantee a D.J. Performance on_____________________ (date) at _____________________________ (Location) for the purpose of ________________________________________________(event type) for ______________________________ (customers name) __________________(phone #) for a total price of ______________________with a ____________________deposit which counts towards the total price. This service agreement also provides for the use of adequate sound equipment, a standard selection of music and lighting. The balance of the total price is due immediately after services are rendered. The deposit is only refundable one month in advance by the retaining party upon cancellation notification. Deposit refund after that time is at the discretion of Dave Carmona. Deposit becomes payable as this service agreement is signed by both parties and on the date indicated below.
